In this webcast, Got Transition provides an overview of health care transition and describe its importance as it relates to students with special health care needs and their families. We review resources created by Got Transition including A Family Toolkit: Pediatric-To-Adult Health Care Transition and Health Care Transition Readiness Assessment for Students with an IEP as well as discuss strategies for educators to incorporate health care into transition planning.
Presenter Bio:
Annie Schmidt is responsible for conducting research and policy analysis and is involved with a variety of projects related to health care transition, including research on value-based payment and Medicaid managed care contract analysis. She has previous research experience in the fields of mental health and tobacco control. Ms. Schmidt received her master’s degree in Public Health from the Gillings School of Global Public Health at the University of North Carolina-Chapel Hill.
