This webcast will focus on transition care for patients enrolled in the Pediatric Care Navigation Program at Niswonger Children’s Hospital in Eastern Tennessee. This webinar will discuss how the Transition Coordinator aids pediatric patients in transitioning to adult care.
 

Presenter Bio:
Tara Chadwell is the assistant vice president of Child Health Programs at Ballad Health. Working alongside both Ballad Health’s population health team and the Niswonger Children’s Network team, Chadwell is responsible for many of the programs that foster health, wellness, and safety for children in the Appalachian Highlands. Under her leadership, Ballad Health has launched Strong Starts, the Children’s Resource Centers at Niswonger Children’s Hospital and Johnston Memorial Hospital, and, most recently, a pediatric care navigation program to support chronically ill patients seeing pediatric subspecialty providers. Chadwell began her career with Ballad Health in 2013, working in health advocacy, and that has remained her primary focus throughout her career. Chadwell is passionate about women and children’s health and wellbeing beyond the workplace, sitting on the boards of the Regional Child Abuse Taskforce, Sullivan County Imagination Library and the YWCA of Northeast Tennessee and Southwest Virginia.
