The Tennessee Healthy Transitions Initiative grant is funded by the Substance Abuse and Mental Health Services Administration (SAMHSA) and is administered by the Tennessee Department of Mental Health and Substance Abuse Services (TDMHSAS). Our vision is to ensure every young adult across Tennessee has access to community-based mental health, emotional wellness, and behavioral health services. Our goal is to work within Tennessee’s system of care to support the unique individualized needs of young adults. We aim to empower and equip young adults with the tools to become the primary decision-makers of their lives and their mental health journey.

Presenters:

Krystal D. Fortney, MA, LPC-MHSP, ACS. Krystal Fortney is a licensed professional counselor with a designation in mental health for the state of Tennessee; she is a National Certified Counselor and a Mandatory Pre-Screening Agent. Currently, Krystal Serves as the Healthy Transitions Project Director for The Tennessee Department of Mental Health and Substance Services.

Sabrina Fillers, Youth & Young Adult Coordinator, TDMHSAS. Sabrina Fillers serves as the Youth and Young Adult Coordinator with the Department of Mental health and Substance Abuse Services (TDMHSAS). She has worked in peer support for the last five years and is a certified Young Adult Peer Support Specialist.
